Default Former Employer still reporting wages!?

I was working as a CPA for a lady back in 2006 and to receive my paychecks the lady would have to call it in her "office" to give the hours that ive worked for that week so the office could do payroll and the checks would be sent to her which then she would give to me. fast-forward 4 months After realizing that i couldnt commit to the job, i decided to quit/not show for work and figured that was the last i would hear from her. Since March 07 I found a full time job and was recently laid off in Oct 08. While claiming unemployment the DUA office discovered that the lady is still reporting me to her office and receiving my checks in the amount of 4K. Now im making the ASSUMPTION (what else would she be doing with my checks?) that she is cashing my checks. Im so shocked i dont know where to begin the process of correcting my wages with the SS office.. etc. any ideas?
